Q: Does the H Street Clinic take walk-ins?

A: No, please make an appointment by calling us at (909)381-0803.


Q: I am not a U.S. citizen, and I am undocumented. Will I be deported if I seek care at the H Street Clinic?

A: No, your documentation status does not matter to us. Please come in to the H Street Clinic and get treatment without worries of being deported.


Q: What is CHDP?

A: CHDP stands for Child Health and Disability Prevention. It is a preventive program that delivers periodic health assessments and services to low income children and youth in California.

Q: What does CHDP offer?

A: Growth and development checks; all needed shots; dental, vision and hearing screenings; nutrition check up; health education; tobacco education; and WIC referral for children up to 5 years old.

Q: Why do my children need check-ups?

A: A complete health check-up can catch medical, dental, or mental health problems before they get serious. It also allows you time to ask questions, and necessary shots can be given.

Q: Who qualifies for this program?

A: You qualify if you are a Medi-Cal recipient and your child is between the ages of 0-21. You also qualify if your income is equal to or less than the federal income guidelines.

Q: What is FPACT?

A: This program provides family planning services to low-income men, women, and teens at no cost.

Q: Why should I enroll?

A: FPACT can help you:

  • Prevent an unplanned pregnancy
  • Protect your reproductive health so you can have a healthy baby when you're ready
  • Find out about birth control methods
  • Get the education, counseling and treatment that you need

Q: Am I eligible?

A: There are many ways to be eligible for FPACT. All you have to do is fill out a simple form to see if you qualify.

A: H Street Clinic is a FPACT provider.
